Document Verification Appointment

Service Description

To complete your scholar’s registration in person, please bring the following documents. These items are required to verify your child’s eligibility and residency for enrollment at SR1 CPSA: Original Long-Form Birth Certificate – to verify your scholar’s age and identity. Mississippi Immunization Form 121 – proof of current immunizations as required by the state. Parent/Guardian Photo ID – acceptable forms include a driver’s license or state-issued ID. Legal Guardianship Documentation – if applicable. Two Proofs of Residency – showing the parent or guardian’s current address. Acceptable forms include: Driver’s License 🪪 Utility Bill 💡 (electric, water, or gas) Voter Registration 🗳️ Vehicle Registration 🚗 Please ensure all documents are current and original copies. Copies or photos cannot be accepted for verification.
